System Safety

QI-Solutions Teams’ engineering and analytical disciplines ensure that safety is considered in all aspects of design, development, operation, maintenance, and modification of systems and platforms.

a. Under N63394-98-D-1106, QI-Solutions Team supported NSWC Port Hueneme Division (NSWC PHD) for all variants of the Standard Missile Program, providing in-service engineering, test and evaluation of missile system components; and the development of all technical documentation in paper and digital formats, and design and development of test sets and prototypes.

b. Ordnance safety assessments, ordnance safety briefs, and post-loadout assist visits. We also ensure that handling and transporting of weapons is performed using the correct equipment and provide OJT as needed.

c. In support of PMS 405, under our GSA PES contract, QI-Solutions’ Team prepared briefing materials and participated in presentations to the Weapon System Explosives Safety Review Board (WSESRB) to obtain Board concurrence to install the AN/USQ-T46(V) Battle Force Tactical Training System, including the AN/USQ-T48(A Tactical Simulator/Stimulator System (TSSS), aboard all Ship Self Defense System (SSDS) MK 2 capable ships and utilize the TSSS with all radars, including fire control radars. Reviewed System Trouble Reports (estimated at 75/year) to determine safety impacts. Prepare and provide hazard analyses such as that prepared for the proposed shock qualifications of the BFTT NavSim Indicator and Control Panel. Reviewed and provide analyses of operational safety concepts papers and Technical data Packages. Conduct and interpret Combat System safety analysis, develop and maintain System Safety Program Plan (SSPP). Develop, update, track, monitor, report Hazard tracking databases and Hazard Action Request (HAR) and other required safety program documentation. Reviewed and analyzed system hardware/software development, modification, and/or change documentation to determine impact to system safety. Present System Safety Program specifics to Software System Safety Technical Review Panel (SSSTRP) and Weapon System Explosives Safety Reviews Board (WSESRB) and provide principal liaison to other Combat System activities for coordinated safety program requirements. Develop DOD Information Technology Security Certification and Accreditation Process (DITSCAP) Interim Authority to Operate (IATO) documentation and ensure DITSCAP discrepancies are identified, addressed, and corrected. Developed test plan for system DISTCAP. Developed Technical Data Packages encompassing O&SHA, FMECA and SSHA, FTA, and PHA.
